Pharmacist

University Health San Antonio, TX (Onsite) Full-Time

POSITION SUMMARY/RESPONSIBILITIES

The Clinical Pharmacy Specialist II is responsible for the provision of comprehensive clinical pharmacy services related to a specific area of practice to optimize safe, efficacious, and cost-effective drug therapy in accordance with all regulatory agencies, including but not limited to The Joint Commission (TJC), Occupational Safety for Health Administration (OSHA), Medicare/Medicaid standards, and Texas State Board of Pharmacy Rules and Regulations.  The Clinical Pharmacy Specialist II serves as a pharmacotherapy expert member of an interdisciplinary patient care team to provide direct patient care, perform pharmacotherapy consults, provide medication-related education to patients and other interdisciplinary team members, participate in quality improvement or research initiatives, and contribute to development of medication-related best practice guidelines and policies related to their specific area of practice.  The Clinical Pharmacy Specialist II serves as a leader for the department in their respective area of practice, organizing appropriate departmental initiatives and providing education for pharmacy colleagues, providers, and trainees at all levels of practice.  The Clinical Pharmacy Specialist II demonstrates a working knowledge of the operational aspects of the pharmacy department and contributes to the overall departmental objectives as an integrated member of the pharmacy team. The Clinical Pharmacy Specialist II demonstrates an advanced level of competency in a specialty area and will serve as a mentor and role model for Clinical Pharmacy Specialists I in developing clinical and practice management skills for achieving patient care goals.

EDUCATION/EXPERIENCE

A Pharm.D. degree or a Bachelor of Science in Pharmacy from an ACPE-accredited School of Pharmacy is required.    Completion of an ASHP-accredited Postgraduate Year Two (PGY2) Pharmacy Residency with at least 1 year a experience in a specialty area or equivalent experience (PGY1 plus 2 additional years of experience with at least 1 year in a specialty area; or 3 years of experience with at least 1 year in a specialty area, in health-system pharmacy) is required

LICENSURE/CERTIFICATION

Current Pharmacy and Pharmacy Preceptor licensure, or eligibility for licensure, in the state of Texas is required. Certification by the Board of Pharmacy Specialties (as appropriate to specialty), approved certification or advanced degree is required
