Nurse Practitioner

Position Summary/Responsibilities
Serves as a member of the Geriatric Inpatient Consult Service team to address specialized healthcare needs of the aging population. Conducts comprehensive and focused geriatric assessments related to geriatric syndromes and disease states as part of patient-centered care to aid in admission prevention and reduction in hospital length of stay. Performs medication optimization strategies for polypharmacy reduction and de-prescribing efforts. Collaborates with interdisciplinary teams to streamline transitions of care for safe disposition and community provider partnership. Participates in departmental quality initiatives, service line operations, and inpatient and ambulatory care provision.

Education
A Registered Professional Nurse who is prepared for advanced nursing practice by virtue of knowledge and skills obtained through an accredited post basic or advanced educational program of study acceptable to the Board of Nurse Examiners. A Master’s degree in nursing is required. Three years of nursing experience is required and two years of advanced practice is preferred. Adult Acute Care Nurse Practitioner certification or Family Nurse Practitioner certification is required and board certification in Geriatrics is desired.

Licensure
Must be currently licensed as a Registered Nurse in Texas and credentialed by the State to practice as an Advanced Nurse Practitioner. Adult Acute Care Nurse Practitioner or Family Nurse Practitioner certification is required, and board certification in Geriatric Care is preferred. Prescriptive authority and DEA is required.
