Health Equity Manager
POSITION SUMMARY/RESPONSIBILITIES
The Health Equity Manager for Community First Health Plans, Inc. (Community First) serves in a dual role to assist eligible members secure long-term housing and remove barriers to living independently, and assists eligible members with training and coaching on how to acquire the physical, intellectual, emotional and social skills needed to perform specific job tasks.
This role involves orienting members to the appropriate program and providing housing search assistance, individualized tenant education, supportive services to promote member’s self-sufficiency, integration into the community and permanency in housing, and developing relationships with stakeholders throughout the community to promote permanent housing opportunities for members experiencing a housing need.
This role will also establish programs to assist members develop work routines and personal life activities that are critical to achieving successful employment, orienting members to become better adjusted and independent at their job site in order to achieve their career goals.
EDUCATION/EXPERIENCE
A Bachelor’s degree in Human Services, Psychology, Social Work, Sociology or a related field is required. Two years of direct practice successfully assisting and supporting individuals or families who have experienced homelessness to secure and retain permanent housing required. Experience in developing programs to address the needs of individuals with intellectual and developmental disabilities is required. Knowledge and experience with employment training and placement agencies. Excellent verbal and written communication skills. A certified Supportive Employment Specialist is preferred.
