POSITION SUMMARY/RESPONSIBILITIES

      This position will serve as the link between the community, collaborating network partners, University Health providers, and the University Health Mental Wellness Institute in order to increase the utilization of available health and behavioral health programs and maximize the use of existing health services as deemed appropriate.  Assists and guides patients through community health prevention and treatment services to ensure that patients are able to access and keep medical appointments, participate in recommended care plans, and receive appropriate health information about their diagnosis and treatment plan.  Assists and supports community outreach, including, but not limited to, health fairs, University Health-sponsored events, and educational classes. All functions and behaviors are applicable to clinical and community settings.

EDUCATION/EXPERIENCE

High School Diploma or G.E.D. with minimum of 2 years’ experience in the area of health education, public health, patient care, or case management. Community Health Worker Certification or Bachelor’s degree in social sciences or health education or related field of study is preferred. Experience in public speaking and educational presentations demonstrating effective communication skills is required.  English/Spanish fluency is strongly desired.
