Nurse Case Manager - Transplantation Programs
POSITION SUMMARY/RESPONSIBILITIES
Coordinates the interdisciplinary treatment plan of care for patients across the healthcare continuum within a defined population. Facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate health care needs.
EDUCATION/EXPERIENCE
Graduation from an accredited school of professional nursing is required. A Bachelor's degree or Master's degree in Nursing is highly preferred. Two years of acute nursing experience, preferably in Intensive Care, is required. Two years experience in any case management care setting; HMO, Physician Group Model, MSO, or Acute Care is required. Five years nursing experience preferred (Staff nurse II or above). Work experience in case management, utilization review or hospital quality assurance is preferred. A national certificate in case management is preferred.
LICENSURE/ CERTIFICATIONS
Current licensure as a Registered Nurse with the Texas State Board of Nurse Examiners is required. Current American Heart Association, Basic Cardiac Life Support and/or Health Care Provider card preferred.
